Full-time employees of Medina City Schools are eligible for medical, dental and prescription drug insurance.  The District's insurance carrier is Medical Mutual of Ohio (MMO).  Employees who have questions regarding benefits can go on-line to:  www.MedMutual.com or call MMO Customer Service at 1-800-585-2583.

The District's life insurance carrier is MetLife.  Questions regarding life insurance can be directed to Human Resources at 330-636-3013 or 3034.

For questions regarding vacation, personal or sick time, 403(b) deductions or automatic deposit, please contact the payroll department at 330-636-3054 or 3055.

Support Staff Employees who need to contact the School Employees' Retirement System (SERS) may do so by calling 1-866-280-7377 or going on-line to:  www.ohsers.org

For any of the claim forms, print the form, fill in the information, attach any supporting documentation and send to MMO or Medco as appropriate.  If you are unsure which form is needed for your particular situation, please contact Human Resources at 330-636-3013 or 3034.

For the Life Insurance Beneficiary Form, MMO Change Form or Spousal Coverage Form, please print the form, complete the information and send to Human Resources.
